Heavy rainfall caused torrents of water to flow through valleys in the Umm Dafuq area, located 250 kilometres southwest of Nyala in South Darfur State, preventing her rescue. While her relatives desperately tried to raise money to hire transportation, she died from severe bleeding.

Hanan’s case is not isolated. Women in these areas face pregnancy and childbirth complications, leading to repeated deaths, particularly in the localities of Umm Dafuq and Reheid Al-Birdi in South Darfur State. Roads are cut off, and emergency assistance for pregnant women is often delayed. In other parts of Darfur, women are forced to walk long distances, travel on the backs of animals, or be carried on people’s shoulders in search of maternity services. Others are transported on wooden stretchers as they desperately seek hospitals where they can give birth.

With roads becoming impassable during the rainy season and transportation and healthcare services scarce, rescuing these women can become a race against death.

These cases expose the health and humanitarian crisis facing women across the Darfur region as reproductive healthcare services have deteriorated since the outbreak of war in April 2023. Hospitals and specialised centres have been forced out of service, worsening the suffering of pregnant women and depriving them of antenatal care and safe childbirth services.

According to the Darfur Network for Human Rights, a non-governmental organisation that monitors and documents violations against civilians across the states of the wider Darfur region, pregnant women travel long distances to reach the nearest healthcare facility. Most facilities are located several kilometres away from villages and rural communities, while transportation options remain limited.

The network notes that women experiencing obstructed labour often fail to receive life-saving care, resulting in their deaths. According to estimates by the World Health Organization (WHO), approximately 8.1 million women and girls of reproductive age in Sudan require urgent health services, including more than 803,000 pregnant women, amid the widespread deterioration of the country’s healthcare system.

Deteriorating Services

Adam Fadlallah, a resident of Golo in Central Darfur State, said: “My wife passed away just as we were preparing to welcome our first child. This happened because it was difficult to reach the only hospital providing services to the population. The hospital has limited resources and only one doctor performing Caesarean sections, leading to an increase in the number of patients and worsening the suffering of pregnant women, particularly those nearing childbirth.”

He added: “Sadly, conditions for women in villages and rural areas are extremely critical. This has forced them to resort to traditional practices, taking them back to the era of giving birth with the aid of a rope. Such practices have become widespread because reproductive healthcare is limited and difficult to access. Some areas are now completely isolated during the rainy season. Nevertheless, women are carried on people’s shoulders or on the backs of animals while screaming in severe pain as they suffer miscarriages or bleeding.”

Fadlallah pointed out that food shortages are among the dangers worsening women’s health in the region. Inadequate nutrition weakens their resistance to illness and increases the risk of complications before and after childbirth. This is happening amid declining agricultural production and mounting economic and health pressures.

He said pregnant women are suffering from malnutrition without timely responses or interventions, except for assistance from a few organisations whose operations remain limited and fail to meet needs on a wider scale. He stressed that providing food is a particular priority for these women, sometimes taking precedence over other needs.

Risks and Deaths

Meanwhile, Mariam Youssef, a reproductive health worker in Umm Dafuq, said: “During the war, conditions for pregnant women in villages and rural areas have become extremely harsh because midwives are absent from health centres that have closed their doors, despite previously providing services. This has exposed women to the risk of death as they are forced to give birth at home using traditional methods without proper sterilisation.”

She explained that the distance to hospitals and the lack of transportation prevent women from receiving adequate care during pregnancy, increasing the number of deaths.

The reproductive health worker continued: “To save women’s lives, whether during pregnancy or childbirth, health centres in villages and localities must be reactivated. This would help prevent the dangers threatening their lives.”

She noted that approximately 12 deaths had been recorded within a single month, but the figure did not reflect the full extent of the tragedy, particularly because some women never reach hospitals and their deaths consequently go unregistered.

Women’s rights activist Afaf Hamid said: “The ongoing war in the country continues to exact a heavy toll on women and girls, posing a direct threat to their reproductive health. Displacement has further compounded their suffering and made access to essential services increasingly difficult.”

She added that these difficulties intensify during the rainy season as roads become impassable, while healthcare facilities in many areas remain out of service. Obtaining essential supplies has become virtually impossible.

She further explained that women are also being deprived of necessary vaccinations during pregnancy, regular monitoring of foetal development and routine medical examinations. The lack of electricity has also restricted the ability of hospitals receiving patients from villages and rural communities to perform Caesarean sections.

Hospitals Under Strain

Hamid added: “Al-Daein Hospital in East Darfur State is in a dire condition, particularly after being subjected to bombardment. Despite its deteriorating state, it continues to receive patients from several localities while suffering from shortages of doctors and medicines. At the same time, it is overcrowded and lacks a specialised treatment unit for maternity and reproductive healthcare.”

The women’s rights activist believes that international and local humanitarian organisations, United Nations agencies and the relevant authorities must act urgently to provide specialised medical personnel, medicines and life-saving treatments. She also called for the establishment of mobile clinics in villages and underserved areas.

She warned that allowing the situation to continue unchanged would inevitably lead to a humanitarian catastrophe, requiring long-term strategies, including training midwives and expanding healthcare services in villages and rural communities to reduce the risks facing women.

In the same context, social researcher specialising in Darfur affairs, Mohammed Abakar, explained that hospitals that previously provided services before the outbreak of war have continued to close their doors, further worsening the difficulties pregnant women face in accessing safe childbirth, reproductive healthcare and maternal services.

He noted that dozens of women in different parts of the region, particularly in rural communities, have died as a result of miscarriages, anaemia, premature births and obstructed labour.

He stressed that the few remaining hospitals and health centres are no longer able to save women’s lives because of shortages of medical personnel and depleted medicine supplies. He added that Golo Hospital and Trenti Hospital are the only facilities still capable of performing Caesarean sections following the closure of major hospitals.

The family of Hanan Musa, who was experiencing labour pains, was unable to get her to Um Dukhun Hospital in Central Darfur State.
